Note 1 –  Volunteers under the age of 18 must have a waiver form signed by a parent or guardian. Click on the Agreement of Release, Waiver, and Indemnity form. Fill it out and submit online.

Note 2 –  Volunteers, who have never filled out an Agreement of Release, Waiver, and Indemnity Form, on paper or online, must fill one out and submit it online.
